1. Statement of certifying physician
All 4 numbers must be circled
Number 2 must have at least 1 condition circled
2. Prescription for footwear must state
"Extra depth shoes to accommodate diabetic conditions"
3. Prescription for inserts must state
"Multi-density inserts to accommodate diabetic conditions"
(Medicare allows 3 pair of inserts per year)
Must state 2 refills to be eligible for 3 pair
